COLLEGE APPLICATION CHECKLIST

If you are planning on attending DVC or another junior college in California you will not need to fill out a college application or request a transcript. You will need to take an English and Math assessment test in the spring. We will be offering the assessment test on campus in March or you can take it at the Main Campus (Pleasant Hill) or San Ramon Center of DVC. Check with the Career Center for test dates. Once you have taken the assessment test you will sign up for Counseling 105. You will need to complete this course before meeting with a DVC counselor about registering for classes.

In order to simplify the 4-year college application process, we are asking that you complete the following steps before bringing college application material to your counselor.

PLEASE GIVE US AT LEAST 2 WEEKS NOTICE TO COMPLETE THE SECONDARY SCHOOL REPORT AND LETTERS OF RECOMMENDATION.

  • Complete Student Resume Form available in the Career Center or on line @ www.srvhs.org (click Career Center).
  • Have Parent complete Parent Brag Sheet available in the Career Center.
  • For each private school that you are applying provide:
    1. College Secondary School Report (also called Counselor Report).
    2. Stamped, addressed envelope.
    3. Indicate if you are applying early or regular decision and due date of application.
    4. Complete Part 1 of your private school application before you request recommendations.
  • Fill out a green transcript request form available from the registrar, $2.00 each.
    Indicate on the form that you want the transcript to be:
    1. Official
    2. Current Transcript
    3. If the counselor is sending transcript with the school report check Send to Counselor
    4. If registrar is sending transcript directly to school provide a stamped, addressed envelope

You are not required to send transcripts to CSU or UC schools at this time, a final transcript is sent in June to the college you will be attending.

  • Have your SAT and/or ACT scores sent to the colleges to which you are applying. You may go online to www.collegeboard.org or www.actstudent.org to take care of getting scores sent.
  • If you are a student athlete make sure that you are registered with the NCAA Clearinghouse You may go to www.ncaaclearinghouse.net for initial eligibility requirements.


Staying organized and managing your time will help to make this process less stressful. While we are working on our part of your applications, we suggest that you start thinking about your personal statement (required for UC applications) and any other essays that your schools may require. You will also want to get an unofficial copy of your transcript available in the Career Center so that you may use it as a reference when filling out your applications.
